Authorizing a Cremation – How the Process Works
Centre City Cremation strives to make the process of purchasing cremation services as stress-free and uncomplicated as possible. We provide all the necessary paperwork online, including a digital signature, so all you have to do is fill out and submit the forms to our team. In four steps – filling out the required vital statistics form, authorizing the cremation, declaring the disposition of the remains, and, finally, either purchasing the cremation online or arranging to meet with a representative – you can have the cremation arranged in privacy and confidence. STEP 1: Fill out and send the Statistical Information (Vital Statistics) form STEP 2: Fill out and send the Cremation Contract and Authorization for Cremation and Disposition form STEP 3: Fill out and send the Declaration of Disposition of Cremated Remains form STEP 4: Purchasing the Cremation
| STEP 1: Fill out and send the Statistical Information (Vital Statistics) form. The Statistical Information (Vital Statistics) form provides information on the Deceased’s age at death, residence, educational and employment background, family history, and other information. |
| STEP 2: Fill out and send the Cremation Contract and Authorization for Cremation and Disposition form. The Cremation Contract and Authorization for Cremation and Disposition form authorizes the cremation and disposal of the remains. |
| STEP 3: Fill out and send the Declaration of Disposition of Cremated Remains form. The Declaration of Disposition of Cremated Remains form authorizes specified individuals the disposition (scattering) of the remains. |
| STEP 4: Once you have filled out the required forms, you have a choice of either having a representative meet with you or making the purchase directly online. (If you choose to purchase your cremation certificate directly, please keep your invoice.) At this point, we have received your forms along with your digital signature and process the purchase. A representative will contact you to let you know that your certificate is being mailed to you. |
